Notice of Privacy Practices (HIPAA)
Earley Health and Wellness — Notice of Privacy Practices
Effective date: 10/01/2026
This notice describes how your protected health information (PHI) may be used and disclosed, and your rights regarding that information. Please review it carefully.
Who this covers: Earley Health and Wellness and Alicia Earley, FNP-C, providing telehealth services in Arizona, Massachusetts, New York, and Illinois.
How we may use and disclose your PHI
Treatment: To provide, coordinate, or manage your care — for example, reviewing labs, prescribing, and communicating with you through your secure patient portal.
Payment: To process your membership payments and provide receipts or superbills. (We are a cash-based practice and do not bill insurance.)
Health care operations: To run the practice — quality review, staff training, and maintaining your clinical record in our HIPAA-compliant systems (Practice Better, DrFirst ePrescribe).
As required or permitted by law: For public health reporting, to prevent a serious threat to health or safety, in response to a court order, or as otherwise required by state or federal law.
With your written authorization: For any other use — for example, sharing records with another clinician at your request. You may revoke an authorization in writing at any time.
Your rights
Access: Request a copy of your health record.
Amend: Request a correction if you believe information is inaccurate.
Accounting: Request a list of certain disclosures we have made.
Restrictions: Request limits on how we use or disclose your information (we are not required to agree, except for certain self-pay restrictions you request in writing).
Confidential communications: Request we contact you a certain way or at a certain location.
Paper copy: Request a paper copy of this notice at any time.
To exercise any right, contact us at (480) 401-2132 or aearley.fnp@gmail.com. We will respond within the timeframes HIPAA requires.
Our duties: We are required by law to maintain the privacy of your PHI, provide this notice, and notify you if a breach of unsecured PHI occurs.
Complaints: If you believe your privacy rights have been violated, contact us at the number or email above, or file a complaint with the U.S. Department of Health and Human Services. Filing a complaint will not affect your care.
